Grishko makes Ulanova II, this is a shoe designed for longer big toe or second toe.  The widths are generous, you will want to check the heel, it may be on the normal to narrow side.  The vamp is higher and the box is tapered to keep you from sinking into the box.  I have the longer second toe and narrow feet, I can't wear many styles of pointe shoes, so this is something I've spent some time looking into.  Go to Grishko's website and write them a note with what you need, they will respond quickly.  If you have a high arch, it is likely to be less flexible than an average arch, and needs protection.  You may consider ordering a hard shank.  I think if your feet are strong, you will be able to wear them well.  Stay away from any split-sole shoes, you could damage your feet without the right support, including collapsed arches.  Besides, it takes the work out of breaking your shoes and strengthening your feet, you are more likely to get injured.  Ulanova II is a special order shoe, if you need something readily stocked, try Grishko's 2007.  Honest, I don't work for Grishko, but I do dance and I fit pointe shoes for dancers on the weekends.
